An unofficial report stated a pilot was unfortunately killed during the incident. The official report from Civil Aviation Authority of Malaysia stated that the plane was on a night training flight at Sungai Pinjil near Medan Gopeng Ipoh. It took off from Sultan Azlan Shah Airport.

The Air traffic Control Tower received a mayday call at 8:07pm. The aircraft is a Piper 28-161, registration 9M-BAA and is operated by BATS Aviation Sdn Bhd.

The aircraft was piloted by Captain Haji Fajim Juffa and Captain Din Fikri, both are flight Instructors at BATS Aviation. Captain Fajim was a former Lieutenant Colonel Fighter Pilot from Royal Malaysian Air Force and reportedly lost his life in this unfortunate incident.
